Care Manager - Home Care Agency, Hall County and Northeast GA

We are a growing home care provider agency serving adults with disabilities and seniors across Hall County and Northeast Georgia. We are seeking a motivated, compassionate, and experienced Care Manager to join our team and serve as the cornerstone of our client and caregiver experience.

KEY RESPONSIBILITIES
Client Relationship Management:
Serve
as the primary point of contact for clients and their families from intake through ongoing care
Develop individualized care plans in collaboration with the client and family based on in-home assessment
Perform regular client check-ins (in-person and phone) to monitor satisfaction, safety, and evolving needs
Proactively identify and address concerns before they escalate — communicating with families clearly and promptly
Maintain accurate and up-to-date client records in the care management platform
Caregiver Relationship & Staffing:
Build
strong, respectful relationships with caregivers — understanding their strengths, preferences, and availability
Coordinate and manage shift schedules, ensuring consistent coverage and appropriate client-caregiver matching
Respond promptly to call-outs, scheduling gaps, or caregiver concerns to maintain continuity of care
Participate in caregiver onboarding and ongoing performance feedback
Partner with the staffing coordinator to address workforce challenges proactively
Organic Growth & Community Presence:
Identify
opportunities to expand services within the existing client base as needs evolve (hours, days, specialized care)
Build and maintain relationships with referral sources: physicians, discharge planners, senior living communities, and social workers throughout Hall County and NE Georgia
Represent the agency at community events, health fairs, and senior center programs
Convert referrals to active clients through prompt follow-up and a consultative, trust-based approach
Track and report on growth activities, referral sources, and conversion rates
Care Coordination & Quality:
Ensure
care plans are followed and updated as client needs change
Collaborate with family members, physicians, and other care team members as needed
Monitor for signs of client decline, safety risks, or hospitalization risk factors — escalating appropriately
Support compliance with agency policies, state regulations, and documentation standards

QUALIFICATIONS
Required
Minimum 2–3 years of experience in home care, assisted living, memory care, skilled nursing, or a comparable senior care setting
Strong interpersonal and communication skills — both verbal and written; comfortable with families, seniors, and clinical professionals
Demonstrated ability to manage relationships with multiple stakeholders simultaneously
Scheduling and organizational competence with the ability to handle shifting priorities calmly and effectively
Reliable transportation and willingness to travel throughout Hall County and Northeast Georgia
High degree of empathy, professionalism, and discretion when working with vulnerable populations
Preferred / Differentiating
LPN or RN licensure — aide supervision, and higher-acuity client management
Existing referral network in Hall County or surrounding NE Georgia communities (physicians, hospitals, rehab facilities, senior living)
Experience with home care software such as ClearCare / WellSky, Alayacare, or similar platforms
Prior experience in a client-facing growth or sales role within healthcare or senior services

Flexible work from home options available.

Compensation: $38,000.00 - $58,000.00 per year
