Position Summary:
We are seeking a compassionate and detail-oriented Registered Nurse (RN) or Licensed Practical Nurse (LPN) to perform in-home client assessments in compliance with Georgia Department of Community Health (DCH) regulations for private home care services. This role is ideal for a nurse looking for flexible, per-visit work in a community-based setting.
Key Responsibilities:
- Conduct initial in-home assessments of new clients to determine physical, functional, and emotional care needs
- Develop or contribute to the creation of a Personalized Service Plan (PSP) in accordance with Georgia Rule 111-8-65-.10
- Evaluate client’s medical history, medication regimen, chronic conditions, and ability to perform activities of daily living (ADLs)
- Identify if client is medically frail or compromised and ensure appropriate RN oversight, as required
- Document all findings and recommendations in a timely, accurate, and professional manner
- Provide follow-up service plan updates or reassessments as needed
- Coordinate with care coordinators, aides, and family members to ensure service delivery aligns with the care plan
- Comply with HIPAA, infection control, and agency-specific documentation requirements
Qualifications:
- Current, unencumbered RN or LPN license in the State of Georgia
- Minimum of 1 year of clinical nursing experience; experience in home care, geriatrics, or community health preferred
- Knowledge of Georgia home care regulations and DCH rules related to personal care services
- Comfortable working independently and traveling to client homes
- Strong assessment, clinical documentation, and interpersonal skills
- Valid driver’s license, reliable transportation, and proof of auto insurance
- Ability to pass GCHEXS fingerprint background check as required by state law
Compensation & Schedule:

Per-assessment pay model; rates based on type of assessment (initial, follow-up, complex)
